COMAH Risk Assessment is one of the more challenging aspects of COMAH compliance for both Upper Tier and Lower Tier COMAH sites. Often there is a misunderstanding between what the regulatory authorities require versus what a COMAH site presents as a COMAH Risk Assessment. The most common problems are:

• Major accident hazard definition. 
• Failure to demonstrate that all major accident hazard scenarios have been identified.
• Overuse of generic failure rates in the risk analysis.
